Adamantinoma of the Right Tibia Case Study INTRODUCTION: Adamantinoma is low grade malignant tumor of fibroblast origin. Also called as primary epidermoid carcinoma of bone, malignant angioblastoma or epithelial tumor of bone. It was first described by Fischer in 1913.1 It represents less than 0.4% of all malignant bone tumors.2 The exact origin of adamantinoma is unknown, the classic variant is composed of epithelial cells and osteofibrous components. It is more prevalent in men than in women, ratio being 5:4. The common age of presentation is in the second decade, though it can vary greatly between the second to fifth decades.3 The common site of occurrence is the ramus of the mandible. The other uncommon locations include shaft of long bones (97%), mid-shaft of tibia (80%-85%).4 Other rare locations includes the humerus, ulna, femur, fibula and radius but ribs, spine, metatarsal and carpal bones.5 The characteristic features of adamantinoma is slow, gradual development with high chances of local recurrences and also metastases to lungs .6 In this case study, we have presented a rare histological acanthomatous variant of adamantinoma of the right tibia with metastases to the right inguinal region. CASE REPORT: A 45 year old male patient presented with history of pain in right leg since 3-4 months, insidious onset, gradually progressive, aggravated on exercise, relieved on rest and medication. Later patient had no relief on medication. There was no history of trauma or fall preceeding to onset of symptoms. The patient also complained of swelling in the right groin since 1 month, without pain. On examination, the patient was stable. Local examination showed, a swelling of about 5X3cm in the upper end right leg, tender, well defined, hard in consistency, smooth surface, involving the right knee joint line. There was restriction of movement, but range of movements was present. Examination of right inguinal region revealed a 7X6cm firm, non-matted mass, immobile, non-tender, non-reducible, non-pulsatile mass. Local examination of penis, scrotum, anal canal, hernia orifices was normal. Baseline investigations were done, all the haematological parameters were normal, ultrasound abdomen and chest radiograph was normal. Plain radiograph of right tibia showed large central lytic lesion with sclerotic margins, involving the upper end and shaft of tibia without involvement of right knee joint. Supra-patellar amputation of right leg with right inguinal and right external iliac nodal dissection was done. Histopathological examination of the mass was done. The reported was suggestive of acanthomatous adamantionoma. The features are as described, central area of squamous cell nests which were well differentiated with keratinization surrounded by a rim of myxoid cells. Further immunohistochemistry(IHC) study was done to confirm the diagnosis. IHC was also consistent with the diagnosis and confirmed the diagnosis. Positivity was found for cytokeratin(CK), EMA, CK-19, CK-5, CK-6, P-63, and Ki-67 and vimentin. Also metastases to right inguinal lymph node was confirmed. DISCUSSION: The first bone tumor with epithelial characteristic was reported by Maier in 1900, later in 1913 Fischer termed it as adamantinoma. It is more common in men than in women. The age of presentation varies between the second to fifth decade. The uncommon sites includes the shaft of long bones(97%), of which the mid-shaft of tibia being the most common site(80-85%). The symptoms at presentation include pain with localised swelling. Swelling is the most common symptom. Pain may be present due to history of repetitive trauma associated with adamantinoma.7 Adamantinoma is low grade malignant tumor, and can metastasize to loco-regional nodal areas and to lungs at presentation. The patient in the case report was also a middle aged man in the fifth decade with pain and swelling in the right tibia since 3-4 months and swelling in the right groin. Three theories have been proposed for the source of cell of origin, 1) epithelial, 2) angioblasts, and 3) synovial tumors from uncommon ectopic sites. This was done because the tumor tissue is composed of both epithelial and osteofibrous components.8 In different combinations, different patterns are formed. On microscopy, the commonest form is the classical variant, followed by the osteofibrous pattern. In the classic variant, the types are basaloid, spindle cell, tubular, and squamous patterns. In the classical variant, central epithelial component with surrounding zone of fibrous tissue containing few immature trabecuated bone components are seen. In the osteofibrous variant, cental area of fibrous tissue, surrounded by peripheral zone of epithelial components are seen. Spindle cell variant is most commonly seen with recurrent lesion and metastatic lesions. The typical findings of classical variant were consistent in our case as well, which showed a well differentiated keratinizing pattern of squamous cell nests surrounded by a rim of myxoid cells. Immunohistochemistry exhibits positivity for both epithelial and osseous elements. Fibrous components are positive for vimentin. Epithelial components show positivity for cytokeratin(CK) 5, 14, 19; rarely positive for CK 1,13,17. It is usually negative for CK 8 and 18. Also shows positivity for epithelial growth factor, fibroblast growth factor, fibroblast growth factor receptor. Our case had features as described, also positivity was found for p-63, Ki-67 and CK-6, which favour our diagnosis of acantomatous adamantinoma. Genetic studies reveal chromosomal abberations on chr 7,8,12,19.9 Also aneuploidy has been associated with epithelial components.10 There is loss of heterozygosity of p53 gene. Rarely,(11;22) translocations have been reported in case of adamantinoma, and is termed adamantinoma-like ewing’s sarcoma.11 11;22 translocation is not seen is in adamantinoma. Radiological investigations include radiographs, computer tomography(CT), magnetic resonace imaging(MRI). All the modalities are necessary not only to confirm the diagnosis, also to define the extent of involve and the aggressiveness of the tumor. On plain radiographs usually eccentric, lobulated, cental lytic lesions with a peripheral sclerosis is present in the diaphyseal or metaphyseal ends of long bones. These lesion tend to appear on tibia with a typical picture of soap-bubble appearance.12 CT shows osteolytic lesion in the bone. MRI is the investigation of choice. It helps not only in defining the extent of cortical involvement of bone, soft tissue involvement, but also helps in ruling out other etiologies such as ewing’s sarcoma, adamantinoma, osteosarcoma, metastatic disease, fibrous dysplasia of bone.13 The typical features as described in the literature was true in our case as well. Treatment of adamantinoma has no definitive guidelines. Excisional biopsy is preferred rather than curratage of bone. Curratage of bone is associated with recurrence’s, upto 30% recurrence has been reported with curettage.14 Radical excision of the bone is associated with better outcome and prognosis. Limb salvage procedures include en-block resection, followed by reconstruction of the bony defect which includes vascularised or non-vascularised bone grafts, custom made protheses, intercalary allografts and distraction osteogenesis.15 When limb salvage is not possible, amputation is the definitive treatment of choice. The incidence of mortality with adamantinoma is about 17%, and increases to about 20% when pulmonary metastasis is present. In our patient work up was done and right supra patellar amputation was done, with right inguinal and right external iliac lymph nodal dissection was done and histopathological examination further showed metastases to right inguinal region.

"The Story of an Hour" begins with Louise Mallard being gently informed of her husband’s death in a train accident. Produced by David O Selznick, and directed by Victor Fleming, the film was released a few months after WWII began in Europe. Producer, David O Selznick had obtained the movie right to Mitchell’s novel in 1936 for $50,000. The renowned film was shot in Technicolor which was used very rarely in those times. The historical classic movie of the Old South during civil war period boasts of an eternal cast in a timeless, classic tale of love-hate romance. The movie covers the various romantic pursuits of the resolute heroine Scarlett OHara, against the background of historical events. Ashley, the man she loves marries her cousin, Melanine. She finds it difficult to find love during the civil war years and is forced to seek shelter for herself and her family in the plantation Tara. She defends it against union soldiers, carpetbaggers and even against starvation. Scarlet marries Rhett Butler but the marriage fails due to her indifference towards him. She again returns to Tara. Cinematography: Cinematography is the art of making camera and lighting choices when recording photographic images for the cinema.Techniocolour was not widely used when the film Gone with the Wind was being filmed. Moreover, it carried several disadvantages. Selznick borrowed all the seven heavy, cumbersome cameras of the Technicolor Corporation for shooting. Each picture had colour consulant who had the power to reject any colour scheme she felt was incompatible with colour cinematography. Moreover, the technical advisers, who were supposed to assist the cinematographers, had only worked with black and white film. Technicolor required double the amount of lighting for illumination of a scene. Selznick had previously worked with technicolour. He knew that it would be very important in giving Gone with the Wind visual richness needed for a classic drama. Actually, Selznick wanted the film’s colour to have the same impact as the characters emotions. He was so determined in it and went to the extent of dismissing the first cinematographer, Lee Garmes, for choosing a colour scheme which Selznick thought was very much subdued. The next cinematographer, Ernest Haller, was more successful in obtaining more glowing effects. Selznick had realized that shadow formed an important part in a scene’s visual impact. He asked his colour consultant to shoot Scarlett and her father on the hill at Tara. With the farm gleaming brightly in the distance, the ensuing farming effect strongly emphasizes Gerald’s feeling about the significance of the land. Selznick employs this silhouetting to the similar effect in the film’s final scene, when Scarlett stands on the same mount as she comes home to Tara. Another use of shadow is to highlight moments that focus on relationship between characters in the film. It is first used in the form of the frightening shadows Melanie and Scarlett cast on the walls of temporary hospital. Later on, delivery of Melanie’s baby is lighted with silver streams of light. The darkness in the scene renders it more personal and gives it a powerful simplicity. (Fleming, 1939). The use of Matte painting was another technique which Selznick adapted from black and white film to Technicolour. When a shot is filmed, the area to be painted in afterwards is covered with black matte paint on a glass screen positioned in front of the camera. Afterwards, a colour scale illustration of the omitted portion is shot to cover the blacked out area. Clarence Silfer, the special effects cinematographer of Gone with the Wind used the technique to complete a number of sets that were only partially finished. To my great surprise, when I did that, the spell-checking program highlighted it again and put the hyphen back in! You say look it up? A very good idea, but if a spell checker can’t make up its mind, can we rely on different dictionaries having the same spellings as each other? That’s puzzling! But take solace in the fact that spell-checking programs will never prompt you to misspell a word; they just might prompt a variant spelling. And, no, dictionaries don’t always agree on the best variant, but they never lead one astray. Just use a single dictionary (and, if you write for a client or a company, find out which dictionary it prefers). 3. â€Å"The less-traveled road† is correct. Towards the end of the film, when Deckard and Rachael are fleeing, Decker finds a tiny unicorn, a Pegasus, if you will, in the hallway leading to the elevator. C P Snow and the Second Law of Thermodynamics Essay The scientist and the literary intellectual represent two cultures that are drifting apart from each other to such an extent that each is becoming increasing ignorant of and alien to the other, and because they must represent a body of knowledge as a whole, the consequence is that, though specialization, both the scientist and the intellectual are becoming effectively ignorant. Analysis: Though C P Snow claims to be speaking from a common ground between the two cultures that he envisages, I would argue that he is squarely placed in the scientific camp, and is by no means an intellectual. The manner in which he describes the rift between the two cultures has a distinct whiff of â€Å"shallow optimism† about it, which is the intellectual trait of the scientist. He advocates a simple dialogue between the two camps, which is very much reminiscent of Enlightenment thinking, which, before the advent of modern science, maintained that scientific education was the key to overcoming all social ills, and dialogue is but a means to educate each other. Snow is right in thinking that the two camps had grown apart unawares, and that at one time the cultured man endeavored to keep abreast of knowledge as a whole. But a fundamental point seems to escape him, and that is that modern science entails specialization, and neither does he suspect that it could be the root of the problem. While he acknowledges the existence of specialization in science, he tries to make out that it need not be divisive. His advocacy is of a holistic understanding, and on the strength of this plea he wants to effect a negotiation between the two camps. â€Å"Don’t carry your specializations too far,† he seems to be saying to both the scientists and the intellectuals, â€Å"because both the arts and the sciences are important, and one is in danger of becoming ignorant if one loses complete touch with any one of them. † The propositional content of his plea is correct, but the mistake is to sound it on the platform of modern science, which is divisive in its fundamental aspect. If one is committed to the scientific outlook one must live with specialization. We can take his example about the literary intellectual knowing the second law of thermodynamics as a testing point. He thinks that literary intellectual should at least know this law, which is accepted among physicists as being fundamentally significant. The equivalent feat of for a physicist would be of having read a play by Shakespeare, he suggests. But concentrating on the first point, why should one know the second law of thermodynamics if one will never question its validity? Science functions by constant questioning, and no scientist is ever trained to carry absolute dictates about with him. A literary intellectual may come to it in two ways. He may absorb it as in inviolable dictate, in which case it would not be science at all. Or he may come to it with the proper outlook of the scientist, which is the questioning one. If on the second trajectory, he may either be captivated by the question, or he may deem it not worth his while. If he is captivated, and he remains honest to his intellectual proclivities, then he cannot but pursue the question further, to the detriment of usual literary occupation. But it is more likely that he deems it not worth his while, in which case he returns to the field in which he is proficient and interested. And in due course, through neglect, he forgets how to state the scientific principle at all. If the last is the most natural and likeliest outcome, there is little point in pushing the second law of thermodynamics to the literary man. He has arrived at the status quo of not knowing the law at all, because that is the most natural state of affairs for him. In his situation he has better things to occupy himself with. For Snow to suggest that he ought to know the second law smacks of the arrogance of science, which is an arrogance rooted in naive optimism. Then again, a scientist should only be expected to enjoy a performance of Shakespeare, but certainly not to analyze it. Literary understanding calls for a profound understanding of human nature, which is certainly not part of the equipment of the scientist, who is trained to detect only empirical evidence. To tell a scientist to analyze King Lear would only confuse him, and if he tried too hard it would blunt his scientific perception. Snow would be better advised to consider the underlying philosophy of science, rather than external practice of the separate disciplines. It is a tacit understanding among members of the scientific society (of which literary intellectual are a part) that each practice his own specialization. Only the fruits are to be enjoyed by all, and this is the true egalitarian dimension of atomized science. The notion of â€Å"progress† comes from the understanding that the fruits of specialization confer on all, and it is this notion of progress that binds all members of scientific society. In its original conception modern science was defined as an egalitarianism of knowledge, and apparent loss of this is what Snow is lamenting. But such egalitarianism has not disappeared; it has only become impractical for a single person to keep up with the expanding body of knowledge. But more important than knowledge sharing is the philosophy that underpins it, and this philosophy still unites the particle physicist and the Shakespeare man. In calling for a new, and strained, egalitarianism of knowledge, Snow is only betraying his naivete of the world, which is the characteristic naivete of the scientist daring to speak on the humanities. In 1824 in New York the first reformatory for children was created to protect them from sharing in prison with adult criminals. In 1869 in Boston (Massachusetts) the court hearing to deal specifically with cases of inconsistencies juveniles was first organized and it was implemented the first experience of the Terms of Probation (educational supervision), becoming later one of the most common and, according to Americans the most effective methods of treatment of juvenile offenders. U.S. Federal Law already contained a requirement of consideration of cases involving minors under the age of 16 years, apart from cases of adult offenders. Later, in July 1899, in Chicago under Illinois Law About abandoned children, street and crime children and about how they are treated the first Childrens Court was established. In fact, in Riyadh Guiding Principles as an essential component of the first and second levels of prevention, as well as, perhaps, to a lesser extent, prevention, third level, reflected many of the rights set forth in the Convention on the Rights of the Child. For example, the normal standard of living and access to the educational system, the positive values instilled in children, are both the rights of the child (in the Convention), and elements of the first level of prevention (in the Guidelines). As to the second level of prevention, in this regard, both documents are based on the concept of the primary responsibilities of the family to ensure the welfare, protection and upbringing of the child, that reinforced the obligations of signatories to Convention countries to assist families in fulfilling this role and at the same time to intervene in cases when parents clearly are not willing or able to assume these responsibilities. And by encouraging the adoption of retaliatory measur es against the offender, to avoid recourse to legal proceedings, and setting as the primary purpose of any such measures the social reintegration of a child, the Convention thus reflects a problem in the third level of prevention, as outlined in the Guidelines. The consequence of such a uniform approach, of course, should be the maximum development of initiatives supported by the community and to the family. This is not a task only for specialists in the field of juvenile justice, and for a wide range of state and public organizations with competence in this area. There is no doubt partly because of the extent of the tasks, responsibilities of the organizations have not yet been identified, and actions unsystematic. So prevention and reintegration in the full sense of the word still remain the weakest links in the chain of activities to improve the system of juvenile justice. In practice, efforts to prevent juvenile delinquency are in matter-of-fact identical to the observance and protection of child rights, that are proclaimed by The Convention. This is not surprising, but it is rarely mentioned. Besides, there are still many specialist debating often in spite of the documentary evidences on terms that although are not in every sense of the cause of crime, but at least increase their likelihood. For example, the poverty the absolute and relative in some circles is still considered to be motivation for crime, despite the fact that the Riyadh Guidelines has officially recognized as a factor at the international level. Scientists have located over 500 genes on the four chromosomes in the fly. There are many advantages in using Drosophila for these types of studies. Drosophila melanogaster can lay hundreds of eggs after just one mating, and have a generation time of two weeks at 21Â °C(Genetics: Drosophila Crosses 9). Another reason for using fruit flies is that they mature rather quickly and don’t require very much space. Drosophila melanogaster has a life cycle of four specific stages. The first stage is the egg, which is about . 5mm long. In the 24 hours when the fly is in the egg stage, numerous cleavage nuclei form. Next, the egg hatches to reveal the larva. During this stage, growth and molting occur. Once growth is complete, the Drosophila enter the pupal stage, where it develops into an adult through metamorphosis. Upon reaching adulthood, the flies are ready to mate and produce the next generation of Drosophila melanogaster. During this experiment, monohybrid and dihybrid crosses were conducted with Drosophila melanogaster. Our objective was to examine the inheritance from one generation to the next. We collected the data from the crosses and analyzed them in relation to the expected results. MATERIALS AND METHODS For the monohybrid cross in this experiment, we used an F1 generation, which resulted from the mating of a male homozygous wild-type eyed fly with a female homozygous sepia eyed fly. At week 0, after being anaesthitized by fly-nap, three males and three females were identified under a dissecting microscope and placed in a plastic vial with a foam stopper at the end. The vial remained on it’s side until the flies regained consciousness so that they didn’t get trapped by the culture medium at the bottom. I would also be sure to remove the flies after just one week to reduce breeding between generations. This experiment caused Mendel’s findings to be more concrete and realistic in my mind. It made the information more than meaningless numbers. The experiment also made me realize how easily biological ideas can be proved. Our results agree with Mendel’s discoveries.
